If you are a WCB claimant, you do not need to complete the form. Your case manager will take care of the referral process for you.

Cancellation and no-show policy

Millard Health’s cancellation and no-show policy is followed without exception. Regardless of the reason, charges will apply for all appointments cancelled, rescheduled or missed with less than 24-hours notice (excluding weekends and holidays).

As the referral source, you are responsible for paying the associated cancellation, no-show and late arrival fees.

Cancellations

A 90 per cent fee will apply to cancellations received with less than 24-hours notice (excluding weekends and holidays).

No-shows

A no-show is when a client fails to attend a service that has been booked and confirmed.

A fee of 100 per cent will apply to all no-shows.

Late arrivals

A 15-minute grace period will be allowed for medical appointments and a 30-minute grace period for other appointments. After this grace period, the appointment will be cancelled.

A fee of 100 per cent will apply to all late arrivals.
