To request a claim file:

• By telephone: Speak with your claim owner or a representative from the Claims Contact Centre to request your file.
• In writing: A written request should include your name, claim number, current address and telephone number.
  

Let us know if you would like the claim file mailed or if you prefer pick it up. This request can be faxed to 780-498-7867 or mailed:
